We strongly prefer a formal consultation to be conducted
with the presence the partner prior to scheduling the reversal.
A detailed questionnaire and a focused physical examination allow
us to better appreciate each patient’s condition, anatomy
and particular requirements during the reversal procedure. Typically,
the couple’s many questions are best answered during the
consultation and if needed, with a follow up phone call afterwards.
Following the consultation, procedure scheduling
and other proceedings may be conducted via telephone, fax or e-mail.
For those from afar, we will be happy to conduct
a phone interview and schedule the procedure with a pre-op visit
the day prior to the procedure. We ask that one download the patient
registration, male patient & male infertility questionnaire
and fax the completed forms to us prior to the phone interview.
Procedure may be scheduled Monday through Friday
with 7:30 AM as our preferred
starting time. Please keep in mind that we will try our best to
schedule your procedure on the dates you have selected, however,
elective surgery scheduling is also predicated upon the availability
of operating room, its staff and the surgeon. By planning ahead
and keeping to the schedule, one can avoids the hassle of cancellation
and re-scheduling entirely. If your schedule is less than flexible,
we suggest that you plan ahead by at least 4-6 weeks so we may
secure the desired operating room time slot for you.
As with all non-covered elective surgeries, vasectomy
reversal is a pre-paid procedure.
Please refer to the section on financial policy for payment information
and cancellation policy.
